SmartLab App terms & conditions for Freelancers 

 

 

 

OVERVIEW

 

 

SmartLab desire to retain the services of phlebotomist through SmartLab’s phone Application, and WHEREAS, the Freelance Phlebotomist desire to render services to SmartLab, upon the terms and conditions of SmartLab.

 

 

Scope of Work

 

Freelance Phlebotomy shall work in their free time through the SmartLab Application;entering the patient’s house or office of the designated location in the SmartLab Application for a  specimen collection.; Performing blood collection and receiving Urine and stool samples. Freelancer shall Identify patients and label samples properly and accurately. The Freelance Phlebotomist shall transport collected samples to SmartLab within drop-off time. Other non SmartLab related-activities are not allowed to be performed by the freelance phlebotomists.

 

Termination

 

SmartLab shall have the right to terminate the agreement after providing notice to the second party for the following reasons:

·         Medically Unfit for the job.

·         Less than 4 stars was obtained thru the Application and a Negative result of evaluation was obtained.

 

SmartLab Shall terminate this agreement without providing any notices to the second party for the following reasons:

·         Convicted with dishonesty and crime.

·         Falsification or expiration of documents submitted.

·         Upon death.

·         Committed mistake/s that could be threatening to patient’s life.

·         Any behaviour/s that might affect the SmartLab’s reputation.

·         Engaging in unlawful and unethical practices.

·         Disobeyed company’s Confidentiality and privacy essentials.

·         Any illegal actions of missing items, once proven, shall be subject to disciplinary actions and may possibly become grounds for ending the service with SmartLab.

 

Restrictions

 

 

Freelance Phlebotomists shall NOT obtain or do any of the following during the term of this agreement:

 

·         Explain the past and present results of patients.

·         Disclose results of the patient, their family members and relatives.

·         Perform other procedures not indicated for them to do.

·         Promote other laboratories and their services.

·         Utilize the equipment, devices, items and supplies of other company when performing SmartLab Application work.

·         Disclose the name of the other company/facility/organization they are also working with.

·         Give negative feedbacks about SmartLab, the management, the services, personnel and other co-workers and freelance phlebotomists to the clients and must not be disclosed to any social media platforms.

·         Use any information regarding the SmartLab business to benefit oneself or other companies.

·         Any person or someone related to the SmartLab authorized Freelance Phlebotomist who does not have any Freelance Phlebotomist file in SmartLab shall not be authorized to enter the patient’s house or office, perform specimen collection and even assist the Freelance Phlebotomist during the collection.

·         Any mistake or substandard quality of procedures performed, if committed it shall be documented, investigated and shall be given corresponding sanctions.

·         There shall be no conflict of interest and no conflict of commitment that should arise while working for SmartLab Application, likewise while they are working for the other companies.

·         Freelance should not allow others to use their own ID badge. if so, this will be a possible ground for termination of service for SmartLab.

 

 

 

Confidentiality

 

 

Freelance Phlebotomist shall not share information or talk to people outside of the health care team regarding a client’s private health information. Only team members involved in the care of the client may have access to the client’s health information. Freelance Phlebotomist always carry out utmost care when dealing with patient information.

 

 

Payment and Fees

 

Freelance phlebotomist shall be paid in regarding the following criteria:

 

·         Right Patient identification and Labelling;

·         Right amount of Sample collected;

·         Right tube/container used;

·         On-time Drop-off of specimen to SmartLab; And

·         Passed SmartLab criteria for specimen acceptance.
